Sanford Health Marshfield Clinic (MCC)

The Sanford Health Marshfield Clinic (MMC) is a rural tertiary-care medical clinic with over 1,600 providers across 170 medical specialties that adjoins a 300-bed teaching hospital and children’s hospital. The clinic serves Wisconsin and Michigan’s Upper Peninsula. Behavioral health services are provided through the Psychiatry and Behavioral Health Department, the Children’s Hospital, and the Department of Pediatrics. Behavioral health services include individual psychotherapy, psychological assessment, psychiatric management, and interdisciplinary consultation to provide an exceptional integrated healthcare experience. The provider team includes: psychologists, psychiatrists, psychiatric nurse practitioners, registered nurses, and master level therapists.

Description of Rotations:

This site would provide training in clinical child and pediatric psychology.

Interns can choose from 1 of 2 major tracks:

• Six month rotation in outpatient therapy/6 month rotation in outpatient testing
• Six month rotation in general Children’s Hospital/6 month rotation in outpatient therapy

The following minor rotations will be available:

• PCBH
• Outpatient Therapy
• Outpatient Testing

Major rotations and minor rotations will be constructed to ensure interns are able to follow some outpatient therapy cases for the full 12 month internship.

OUTPATIENT TESTING ROTATION

The intern will obtain psychological assessment experience through the Department of Psychiatry and Behavioral Health Psychological Testing Clinic, The Pediatric Psychiatry and Behavioral Health ADHD Clinic, and The Autism Clinic.

Department of Psychiatry and Behavioral Health Psychological Testing Clinic
Through the psychological testing service, interns will administer and interpret comprehensive psychoeducational and psychological evaluations to assess for anxiety, depression, thought disorders, personality traits, atypical behaviors, ADHD, and learning disorders. Interns will become proficient at administering and interpreting cognitive, academic, executive functioning, projective, and personality measures. They will learn to combine testing data and relevant background information into an integrative report. Interns will become skilled at communicating the results to children and families, the child’s psychotherapist, medical professionals, school personnel, social service agencies, and other community agencies.

Pediatric Psychiatry and Behavioral Health ADHD Clinic
Through the Pediatric Psychiatry and Behavioral Health ADHD Clinic, the intern will participate with child psychiatrists in the assessment of mood and behavioral concerns with special emphasis on ADHD.  Interns will become proficient at interpreting cognitive, academic, executive functioning, emotional/behavioral measures and personality measures related to this specialty service. They will learn to combine testing data and relevant background information into an integrative report. Interns will become skilled at communicating the results to children and families, the child psychiatrist, medical professionals, school personnel, social service agencies, and other community agencies.

Autism Clinic
Through the Autism Clinic, the intern will participate on a multidisciplinary team, which includes occupational therapists, and speech-language pathologists, in the assessment of developmental disorders with special emphasis on autism spectrum disorders. Fellows will learn formal and informal assessment procedures, with an emphasis on conducting parent interviews reviewing DSM 5 criteria for an autism spectrum disorder and administering, scoring and interpreting the Autism Diagnostic Observation Scale-2. Fellows will also learn interventions for children and their families related to this specialty service.

OUTPATIENT THERAPY ROTATION

Individual Psychotherapy
The intern works with children and adolescents exhibiting a wide range of emotional and behavioral problems, including attention deficit hyperactivity disorder, anxiety disorders, depressive disorders, somatization disorders and behavior disorders. Assessment includes interview, collateral contacts with family and other professionals, and psychological tests and behavioral questionnaires. The intern conducts diagnostic assessments, conceptualizes the cases, devises treatment plans, and implements interventions.

Interns are trained in short-term therapy involving relatively rapid assessment of patients’ problems and the formulation of interventions lasting five to ten sessions. In addition, efforts are made to provide experience in longer-term therapy particularly for children and adolescents with persistent psychological problems and/or trauma.

CHILDREN’S HOSPITAL ROTATION

In our regional children’s hospital, the intern will be part of our Pediatric Psychology Consult Liaison Service, working within the general Pediatrics Unit with children and their families who are hospitalized for medical reasons.

The goal of the Pediatric Psychology Internship Position is to enhance the psychology intern’s skills in the assessment, consultation and treatment of children in medical inpatient units.

The pediatric psychology intern will learn to conduct assessments for child and family coping capacity for new medical diagnoses/treatment, assessment of pediatric patients following suicide attempts, adherence to medical regimen, pain management, and the degree to which family dysfunction may interfere with medical treatment. The intern will obtain experience with a wide range of medical patients that are admitted to our Pediatric Unit, including patients with diabetes, eating/feeding disorders, functional disorders, cardiac problems, seizures, migraines and other neurological conditions, gastrointestinal disorders, respiratory conditions, and issues of death and dying.

PRIMARY CARE BEHAVIORAL HEALTH ROTATION

The intern will have the opportunity to obtain training in clinical work with children and adolescents within Pediatric Primary Care as part of an integrated primary care behavioral health service. Training opportunities in outpatient specialty clinics within the Department of Pediatrics are also available, including diabetes and cystic fibrosis.
